What is Trinovum?

As one of the hormonal oral contraceptives commonly known as 'the pill', Trinovum is a very reliable medication taken to successfully prevent pregnancy. Apart from the ability to provide protection against undesired pregnancy, Trinovum also regulates periods and reduces premenstrual symptoms.

Trinovum molecules comprise of two active elements, ethinylestradiol and norethisterone. Both of these are the artificial counterparts of the female sex hormones, oestrogen and progesterone. Ethinylestradiol is the synthetic equivalent of oestrogen and norethisterone that of progesterone.

Trinovum is a triphasic contraceptive, which means that each tablet contains a different dose of hormones. The 7 white tablets in the pack contain norethisterone 0.5mg and ethinylestradiol 0.035mg, the 7 pale peach-coloured tablets have a composition of 0.75mg norethisterone and 0.035mg ethinylestradiol and finally, the 7 peach-coloured ones contain 1.0mg norethisterone and 0.035mg ethinylestradiol + excipients.

How does Trinovum work?

Similar to other oral contraceptives, Trinovum acts on the body's biological functions by overriding the menstrual cycle. The levels of hormones in a woman's menstrual cycle change throughout the whole month. The hormones are responsible for the ovaries releasing an egg and prepare the womb lining for pregnancy. If the egg has not been fertilised, at the end of each cycle, the hormone levels drop, which leads to the womb lining being shed in the form of the monthly period.

Trinovum deceives your body signaling that the ovulation has already taken place. This impedes maturation of the egg and its liberation by the ovaries every month.

The synthetic hormones in this pill also increase the consistency of the mucus naturally occurring at the neck of the womb, which hinders the sperm from swimming across the vagina into the womb to fertilise the egg. They also act on the endometrium by altering its quality and making it more fluid thus preventing the egg from implanting there.

How to take Trinovum

Before you start taking Trinovum, it is of crucial importance to understand the benefits and risks involved. Although it is suitable for the majority of women, it is not the ideal contraceptive for every individual patient. For instance, due to its hormonal composition, it is likely to aggravate or accelerate the evolution of certain conditions. It is also important to inform your physician about any drug treatment you are on or planning to follow, including any herbal remedies or cerebrovascular, cardiovascular, epilepsy, pulmonary embolism, chest angina or liver medication, to ensure the combination does not have a damaging impact on your condition.

Trinovum tablets come in a 21-tablet pack and intake must follow the sequence specified thereon, starting with the first white pill marked– '1'. The recommended dose is one tablet daily covering the whole interval of 21 days. If the pill was taken correctly, the 21 days of pill taking are followed by a seven-day break, during which a withdrawal bleeding occurs similar to your natural period. The next pack should be started immediately after the lapse of the seven days off-pill regardless of whether or not the bleeding has stopped. You will still be protected against pregnancy in the seven days of break from pill taking. To help you remember you have to take your pill, it is advisable that you create a pill taking routine at the same time every day. Swallow each tablet with a sufficient amount of water, either before or after dinner.

Starting Trinovum

For optimal results, it is recommended to start taking Trinovum on the first day of your menstrual cycle. Protection against pregnancy is guaranteed from the very first day of intake. No additional methods of contraception are necessary in this case. You can also start taking the pill at any given moment of your menstrual cycle (up to day 5) without needing to use any other barrier methods for full protection. However, if you get your period every 23 days or less, starting on the fifth day may not offer you full protection. You may need to use other methods of contraception during this time.

You must ensure you are not pregnant before starting contraceptive treatment with Trinovum. If you have only just given birth and are not breastfeeding, start taking Trinovum no later than the 21st day after delivery. You will be protected against pregnancy immediately and won't need to use additional contraception. If you start later than day 21 postpartum, additional barrier methods will be needed for the first seven days of pill taking.

After miscarriage or abortion under 24 weeks, it is advisable that you start taking this pill immediately. Protection is guaranteed from the very first day without needing to use alternative contraception concomitantly. If you start more than seven days after miscarriage or abortion, additional contraceptives should be used for the first seven days of pill taking.

What if I miss a dose?

If you miss a pill, take it as soon as you remember it and take the next pill at the usual time, even if that means taking a double dose on the same day. Then continue taking the rest of the pack normally. You will be fully protected against pregnancy without needing to use other methods.

If you miss out on two or more tablets or start the subsequent pack two or more days later than the due date, you will not be protected against pregnancy. It is best to take the last pill you missed as soon as you remember, disregard the other ones and continue taking the rest of the pack as per the usual routine. Additional methods of contraception are recommended for the next seven days.

If after taking the last missed pill there are fewer than seven pills remaining in the pack, take them all and continue with the next pack without a break. No withdrawal bleeding should be expected until the new pack is finished.

If there are seven pills or more remaining in the pack after taking the last tablet you missed, you can safely take the seven-day break from pill-taking with withdrawal bleeding taking place in this interval.

In case you had unprotected sex in the seven days before you missed out on pill taking, you should use emergency contraception.

Trinovum Side Effects

As with other medications, there is always a possibility that you will experience side effects and Trinovum is no different. The side effects associated with this contraceptive tend to be mild in nature if they even occur.

Vomiting and diarrhoea count among the most frequent side effects of Trinovum. You should not be alarmed if you experience vomiting or diarrhoea within the first two hours of intake, though, this means that the pill has not been fully absorbed into your bloodstream. As soon as your condition improves, take your pill as usual. You should still enjoy full protective effect against pregnancy.

If the vomiting and diarrhoea persist for 24 hours or more, pill efficacy may be reduced. In this case, you should continue taking the pill at the same time, but treat that day as if you had missed it.

Other associated side effects may include weight gain, abdominal pain, headaches, migraines, chloasma, a reduced libido, dizziness, stomach ache and bloating, candidiasis, the formation of blood clots in blood vessels (DVT- deep vein thrombosis) and arteries (ATE- arterial thromboembolism), depression, breast enlargement and tenderness, gallstones, jaundice, higher blood pressure, liver dysfunction or changes in periods associated with spotting between normal cycles or painful periods.

Pregnancy & Breastfeeding

If there is any suspicion of pregnancy while taking this contraceptive, you should immediately stop it and consult your physician as soon as possible.

Trinovum is not recommended while breastfeeding, as due to its hormonal composition it may reduce lactation and alter the quality of breast milk. Use of Trinovum is recommended after weaning or at least six months after birth.
