What is Eucreas?

How does Eucreas work?

The two active ingredients in Eucreas are both antidiabetic agents. Metformin decreases the amount of sugar produced by cells in our liver and increases the sensitivity of muscle cells to insulin. This allows the cells to effectively remove sugar from the blood. Metformin also delays the absorption of sugar from the intestines into the bloodstream.

The other active ingredient Vildagliptin is a dipeptidyl peptidase-4 inhibitor and reduces blood sugar levels by preventing the breakdown of incretin hormones. Our body produces these hormones naturally whenever we consume food which stimulates the pancreas to produce insulin as glucose levels in the blood increase. The hormones also reduce the production of glucagon which increases the production of glucose by the liver. By preventing this breakdown, blood sugar levels are controlled.

The combination of metformin and vildagliptin has been shown to be effective in controlling blood sugar levels after meals and between meals.

How to take Eucreas

Eucreas tablets are prescribed twice daily (morning and evening) with a meal and a drink of water. By taking this medication with a meal you can avoid an upset stomach. What if I miss a dose?

Eucreas Side Effects

Similarly to other medications, you may experience side effects when taking Eucreas tablets. In some cases, individuals may experience no side effects when taking this medication.

Some of the common side effects associated with Eucreas include diarrhoea, vomiting, abdominal pain, loss of appetite, dizziness, headaches, nausea, a metallic taste, low blood sugar levels or tremors.

Some of the more uncommon side effects of this medication include joint pain, fatigue, constipation or swelling of the feet or ankles.

Rare side effects associated with taking Eucreas tablets include hepatitis inflammation of the nose and throat, skin reactions, upper respiratory tract infection, lactic acidosis, decreased absorption of vitamin B12 or acute pancreatitis.

Eucreas Precautions

The following precautions should be adhered to when taking Eucreas tablets to treat type 2 diabetes:

  • The drug is not a substitute for diet and exercise. It will only control blood sugar levels but should be accompanied with a healthy diet and regular exercise.
  • Incidence of hyperglycaemia has been reported when the drug is used in combination with sulphonylurea medicines such as gliclazide or glibenclamide or with insulin. Consult a healthcare provider if you experience any symptoms of hypoglycaemia including cold sweats, pale skin, tremors, anxiety, fatigue, confusion, headache, hunger, vision changes, nausea and/or palpitations.
  • The medicine can cause dizziness and should be taken with care when driving or operating machinery.
  • You should ensure your blood sugar level and your kidney function is regularly monitored while taking Eucreas.
  • Alcohol can increase the risk of low blood sugar and lactic acidosis and should be avoided while taking Eucreas.
  • In some rare cases, hepatitis has been reported in people taking vildagliptin. That is why it is recommended that while taking Eucreas, the patient's liver function should be checked before starting treatment and every three months thereafter for the first year of treatment.
  • Few cases of acute pancreatitis have also been reported in people taking vildagliptin and other similar medicines. If you experience persistent and severe abdominal pain you should consult your doctor immediately.
  • Since people with diabetes are at risk of developing foot ulcers and blisters, ensure you follow a  proper foot care routine.
  • Treatment should be temporarily discontinued if you are planning to have an X-ray that involves an injection of iodinated dye. Eucreas should not be taken for 48 hours after the x-ray and until the kidney function has been tested and is declared normal.
  • Eucreas should also be temporarily discontinued 48 hours before surgery under a general anaesthetic.

Who Should Not Use Eucreas?

Eucreas is not recommended for use by people with Type 1 diabetes, diabetic ketoacidosis, decreased kidney function or kidney failure, decreased liver function, heart failure, respiratory failure, shock, severe infections or trauma, dehydration and alcoholism. It is also not to be used by pregnant or breastfeeding women as well as children and adolescents under the age of 18. Similarly, anyone who is allergic to any of its ingredients should not use Eucreas.

Pregnancy & Breastfeeding

Clinical evidence does not prove the safety of vildagliptin during pregnancy and therefore it should not be used. It is better to control diabetes mellitus during pregnancy by using insulin. While metformin may be used if absolutely necessary, Eucreas is a combination drug with metformin and vildagliptin and should not be used during pregnancy.

Metformin passes into breast milk but it is not yet known if vildagliptin does. In any case, Eucreas is not recommended to be used by breastfeeding mothers.

Other medication interactions

It is important to note that if Eucreas is taken in combination with other antidiabetic medicines, there is a risk of an enhanced blood sugar lowering effect. Therefore, it is up to the doctor's discretion and analysis whether two antidiabetic drugs are needed to control the patient's blood sugar level.

In addition, both cimetidine and rilpivirine can increase the blood level of metformin thus increasing the risk of side effects. Therefore, the doctor may need to reduce the dose in such a case.

Other medicines that increase blood sugar levels as a side effect can reduce the effectiveness of Eucreas. These include:

  • Antipsychotic drugs
  • Beta-2-agonists
  • Corticosteroids
  • Glucosamine
  • Oestrogens and progestogens
  • Thiazide diuretics

There is a risk of hypoglycaemia if Eucreas is taken with disopyramide or ACE inhibitors. In addition, MAOI antidepressants may enhance the blood sugar lowering effect of metformin and in such a case, blood sugar levels should be closely monitored. The antihistamine ketotifen in combination with metformin can reduce the number of platelets and should be avoided while taking Eucreas.
